There are several that we have not received. If the address label on The SPIRE you just received has a red check mark, the HIPAA Form we have for you is dated 2012 and is no longer valid. We need a current HIPAA Form from you.
Remember, we can't visit you in the hospital, print your name in the bulletin, or add you to the prayer list when you are ill without your permission indicated on one of these forms. The forms are on the table in Swain Hall and on the reception counter in the office.
